For the purpose of this discussion, we chose to focus on met- ropolitan Atlanta, the 9th largest metropolitan area in the United States with a population of 5,268,860 as of the last census taken in 2010 (1). We looked at Emory University, the city's largest health- care institution, which is comprised of 6 hospitals, 200 clinics, and 1800 physicians. Emory's hospitals are consistently ranked among the best nationwide, with 10 adult specialties ranked in the nation's top 3% (2). Its healthcare system is vast and continues to expand so we thought we could find some answers to our questions here, among the biggest and best national institutions. Two Emory physicians, both of whom are also assistant professors in the School of Medicine, entertained our topic thoughtfully and with candor. A third clinical voice, from outside of this vast system, comes from an Atlanta physician who is, in his own words, an advocate of universal healthcare, and who specializes in providing care for patients with refractory conditions. We noted that Atlanta is located at the nexus of the Bible Belt and its residents, though often touted as progressive because of the city's ethnic and cultural diversity, remain somewhat conventional in attitude in comparison to similar populations within large northeastern or western cities. Change, we posited, may just come about a bit more slowly in a city and a region where conservative attitudes prevail.
