As a second line therapy, or if the patient is relatively hypotensive, 0.5 to 2.0 ml/kg of 23.4% saline solution can be administered through a central venous line 64 (Class IIA, Level of Evidence B). These measures are designed to lower ICP as quickly and effectively as possible, in order to 'buy time' before a definitive neurosurgical procedure (craniotomy, ventriculostomy (Class IIA, Level of Evidence B), or placement of an ICP monitor) can be performed (Table 3 ).
