ST: This is an unfortunate development for the patient, certainly. JF: It makes for a very inefficient and uneven level of care. Corporate decisions determine allocation of services-for example, affluent areas being served by wasteful duplication of competing facilities because of greater profitability. ST: Well, in the Emory University Healthcare system, as we see from Dr. Garcia-Corrada's experience, there are some areas like the Spine Center where very good efforts have been made, but were not completely tenable, largely because of the nationwide emphasis on physician productivity and demand for increased specialization. But endeavors are still being made to provide comprehensive treatment at Emory for patients in such areas as Neurology and the Sleep Center. My general impression though, is that all large hospital systems that have a plethora of satellite clinics may not be able to allocate their resources in the best possible way for the patient. It seems to me that the general American trend is to assertively market medical services directly to the public just as the pharmaceutical companies have been doing for years. We see this phenomenon every night during the ABC, NBC, CBS and other network broadcasts, and it appears that this trend has its very own momentumsomething like a snowball effect that can't be stopped. JF: We have a situation in the US where the healthcare corpora- tions provide services to generate profit, but the bottom line is that profit supersedes patient care. Their advertising budgets aren't designed to improve care. ST: In the situation you describe, where does this leave, let's say, the chronic pain patient who needs complementary services such as biofeedback, or depression treatment, whether talk therapy or cognitive behavioral therapy (CBT)? JF: It leaves the chronic pain patient, after multiple expensive pro- cedures, frequently no better off. Without an integrative approach, specialists provide their unique services. If they help, great, if not, the patient is referred to the next specialist, until resources are exhausted. All this time the corporation is generating profit. The goal is not to provide the most cost-effective care. Complementary services are often just an afterthought. ST: I see that you have cut right through to the chase. JF: Right. If you're a businessman doctor or a hospital administra- tor, there's no incentive to provide a more cost-effective treatment or a procedure that has less adverse effects or side effects, because you're not making money on it. What people do for acupuncture is look on their own for alternatives because if they go through the system and they get the conventional treatment and it doesn't work, if they have the resources, they're going to look for something that does work. The only way they find acupuncture is if they look. It's not going to be presented to them by healthcare givers as an alternative option, with some exceptions. JGC: It's difficult for the patient to navigate right now. The patients go to a traditional doctor and are still left with chronic pain. What do the patients then do? Maybe they try a chiropractor or maybe they hear about an acupuncturist, or an herbologist, and they're kind of off on their own, really trying to feel their way through a complicated national healthcare system they don't know. ST: Where does this leave allied health professionals, the physical therapists, the acupuncturists, the chiropractors, the pain psychologists? JF: If they are part of the corporate system and their services are reimbursable by insurance, their services are utilized. For those outside the system, it's very difficult. Even if what you provide is better, safer, and cheaper than conventional treatments, it's very hard to compete with the large well-funded institutions selling their well-advertised services. ST: I see. Well, this is twisted around and I think it's making my brain swivel. JF: It's challenging your idealism. Opportunities for acupunctur- ists are mitigated because of profit. Capitalism is not the best system for providing healthcare for our citizens.
