Three days later, both the sore throat and earache were worse, and I had pain throughout the right side of my face. So I returned and saw a different doctor. I got another 90-second exam (at most), was told nothing could be done other than changing the antibiotic, and again was left wondering whether the doctor was done and I could leave. I got a stronger antibiotic (amoxicillin). Neither doctor had even felt for swollen glands. This was a Monday. By Friday, all previous symptoms were worse, and I had such extreme vertigo and nausea that I couldn't drive and had to carry a garbage bag just to walk from room to room. So I went to a Little Clinic at and waited about an hour to see a nurse practitioner, who spent about 20 minutes with me. In that time, she asked several questions about my symptom progression, looked more closely into my ears and nose than either previous doctor, felt swollen glands in my throat - and then explained what she had seen. She said I did have an ear infection and needed to stay on the amoxicillin, but noted that it was not clearing up because it was caused by a sinus infection that amoxicillin couldn't fully cover. She prescribed an inexpensive steroid for inflammation, as well as ear drops and recommended Dramamine 2 to cut my nausea. She said to resume my Zpac if I still had sinus infection symptoms after the amoxicillin ran out, spoke to me about possible drug interactions, and told me that she would check on me by phone in a few days - but noted that the next step, if needed, would be an ENT. Less than 24 hours later, I am already starting to feel better, and - most of all - I feel some confidence that I know what is going on because someone has actually paid attention. I understand that is not trying to offer elite healthcare, and I understand that $99 is not a huge amount for an office visit these days. But it is quite a lot for a 90-second, slipshod diagnosis that extended an illness that could have cleared up faster with a little more thoroughness and compassion.
